The healthcare sector continues to face increasing pressure to deliver high-quality services due to rising service quality expectations, workforce challenges, and heightened accountability for outcomes. In this context, leadership is widely recognised as a key determinant of service quality because it shapes employee behaviour, coordination of care processes, and perceived service quality within hospitals. This study examined the influence of transformational leadership styles on service quality in private level IV hospitals in Nairobi County, Kenya. The general objective of the study was to determine the influence of transformational leadership styles on service quality in private level iv hospitals in Nairobi County. The study was grounded on Transformational Leadership Theory, SERVQUAL Theory, and Social Exchange Theory, which collectively provided a framework for explaining how leadership behaviours influence service quality outcomes in healthcare institutions. The study adopted a descriptive cross-sectional research design targeting 81 private level IV hospitals in Nairobi County. Data were collected from heads of departments drawn from clinical, diagnostic and technical, public health and allied, and administrative and support cadres. The findings indicated that all four transformational leadership styles positively influence service quality, with individualised consideration emerging as the most significant predictor. The study concludes that transformational leadership is a critical driver of service quality in private hospitals, particularly through mechanisms that enhance staff motivation, innovation, and employee development. Methodologically bounded by a cross-sectional design and its reliance on internal key informants, the study's generalisability is constrained to similar private institutional settings. The findings are expected to inform policy, improve managerial practice, and contribute to academic literature on leadership and healthcare service quality in developing country contexts. The study recommends strengthening leadership development programmes in healthcare institutions and integrating transformational leadership competencies into hospital governance and regulatory frameworks.
