Abstract

The rapid advancement of digital health technologies presents significant opportunities to transform healthcare delivery, particularly in low- and middle-income countries (LMICs) such as Kenya. E-Health startups increasingly leverage these innovations to improve healthcare accessibility, quality, affordability, and efficiency. However, many of these ventures in Kenya encountered challenges in scaling and sustaining operations beyond the pilot stage. This study examined the success factors that influence the growth of e-health startups in Kenya through the lens of the Service–Technology–Organization–Finance (STOF) model. A qualitative phenomenological approach was adopted, involving in-depth interviews with founders, senior leaders, and unit managers from nine purposefully selected e-health startups across Kenya. Data were transcribed, organized in NVivo, and analysed thematically. The findings revealed that growth is driven by four interdependent domains. Service factors such as accessibility, affordability, user-centred design, and trust-building mechanisms enhanced user adoption and retention. Technological factors—including interoperability, data security, user-friendly interfaces, inclusive and adaptable digital infrastructure—enabled scalability and efficiency. Organizational factors, particularly visionary leadership, strong team culture, talent development, and strategic partnerships, were essential for sustaining innovation. Financial factors such as financial access & functional revenue models, diversified income strategies and pricing flexibility, financial management &long-term sustainability balance, determined long-term viability. The study concludes that the success of Kenya’s e-health startups depends on their ability to integrate user-focused service design, inclusive and secure technologies, resilient organizational structures, and sustainable financial strategies. Strengthening these four domains enhances scalability and supports Kenya’s broader digital-health transformation agenda. The study recommends further quantitative research to validate these findings across a wider range of e-health ventures and comparable LMIC settings.
